Before you start, have these three things ready

Most delays happen because a blood test report is incomplete. Getting these ready first means your prescriber can review your assessment straight away, with no back and forth.

1

Your full blood test report

A photo of each page, or the PDF. Typed numbers are not enough, because the prescriber needs the laboratory’s reference ranges and the report date.

  • Every page shows the laboratory name, the date and the reference ranges
  • The full blood count page (haemoglobin, MCV, platelets)
  • The ferritin / iron studies page
  • Taken in the last 6 weeks (up to 3 months may be accepted)
2

Your weight today, in kilograms

Your dose is calculated from your actual weight, and we reweigh you on the day of treatment. Weigh yourself this morning if you can. If your scales show stones and pounds, convert it here:

=– kg

1 stone = 14 lb = 6.35 kg. Enter the kilograms figure on the form.

3

Your medicines and history

A list of everything you take, including supplements and anything bought over the counter, plus any allergies. You will also be asked about previous iron tablets or infusions and a short set of safety questions.

What a complete report looks like

Page 1 — Full blood count ✓Example Laboratory · Reported 12 Sep 2026
Haemoglobin 104 g/L (115–165)
MCV 76 fL (80–100)
Platelets 310 x10⁹/L (150–400)
Page 2 — Ferritin / iron studies ✓Example Laboratory · Reported 12 Sep 2026
Ferritin 9 µg/L (15–200)
Transferrin saturation 11 % (20–50)
CRP 2 mg/L (0–5)

Illustrative example only. Your report will look different, but every page must show the same three things: who ran the test, when, and the normal range next to each result. A screenshot of the NHS App results page is fine if all three are visible.

About the medicine we use

The same class of iron used in NHS clinicsFerric carboxymaltose is one of the intravenous iron preparations widely used in NHS hospital infusion units, given by the same route and with the same monitoring standards.
